How to use this checklist

Work through the checklist sequentially for each new driver before their first dispatch. Items marked required have no acceptable substitution under 49 CFR Part 391. Print the checklist, check off each item as you complete it, and keep a dated copy in the driver's DQ file as documentation that a file audit was conducted.

Ongoing maintenance

The DQ file is not a one-time setup — it requires ongoing upkeep. Annual MVR and annual review must be completed within 12 months of the previous one. Medical certificates expire (usually every two years, shorter for some conditions). Clearinghouse limited queries must be run at least once per calendar year for each active driver. Self-audit use

This checklist also works for periodic self-audits of existing DQ files. Compare each item against the documents in the file. If any required document is missing or expired, note it and set a deadline for resolution. FMCSA compliance audits and roadside inspections can request DQ file documentation — a proactive self-audit reduces the risk of violations.

Before you use this template

Treat this page as a working checklist, not a substitute for your carrier's written policy. Add your company name, DOT number, driver name, dates, and the name of the person completing the review before filing a copy. If a checklist item does not apply, mark it that way and note why; a blank field is harder to explain later than a short, dated note.

Keep the completed copy with the underlying evidence: query confirmations, MVR receipts, medical certificates, test results, signed acknowledgments, or other documents named in the checklist. Do not backdate missing records. If you discover a gap during a self-audit, correct it on the actual correction date and keep a note showing what changed.

For multi-driver fleets, save one completed copy per driver or vehicle record rather than keeping a single shared checklist. That makes later reviews cleaner and helps a new manager see exactly which file was checked, by whom, and on what date.

Notes

  • Retention: most DQ file records must be kept for 3 years after driver leaves.
  • Drug test records: negatives 1 year; positives/refusals 5 years.
